A Study on the Behavior of Insulin Self-Injection in Diabetic Patients

Abstract
Diabetes is one of the world-wide distributed chronic diseases resulted in metabolic deficiency. The frequency of it in Korea is increasing according to the rising in the standard of living. Insulin injection has been successfully applied for medical treatment diabetic patients. However, many problems have been found in insulin injection, because patients have to carry out the injection of insulin through life. To establish the education programme for effective insulin self-injection, the author have surveyed the situation of self-injection in diabetic patients. Six general hospitals were randomly selected for sampling of data. A survey has been made on 162 patients who were out patients of the internal department or patients in the diabetic clinic from September 17 to 30, 1992. A questionnaire was developed through the preliminary survey and paper study. Percentage of data, t-test and ANOVA using SAS program were applied. The results are as follows : 1. Situation of insulin self-injection in diabetic patients : The response to the questions on the behaviors of insulin self-injection showed the highest level in sterilizing syringe(4.639) and the lowest level in the management to the hyperglycemia(3.117). 2. Situation of self-injection depending on general factors : Significant difference was found between recognition of injection time and monthly income (F=2.754, p=0.0307). Significant difference was also found between the management to the side effect and the standard of education (F=2.734, p=0.310). 3. Difference in situation of insulin self-injection related to characters in diabetes : Recognition of the sort of insulin injected had a great influence on the situation of self-injection in quantity of insulin injected(F=9.088, p=0.00002), time of injection(F=8.172, p=0.004) order of medicinal mixing(t=2.14, p=0.034), and visiting hospital (t=5.38, p=0.000). Educated experience about diabetes also had a great influence on the situation of self-injection in quantity of insulin injected(t=2.78, p=0.0008), time of injection(t=4.08, p=0.000),orientation 49 of injection(t=2.14, p=0.034),visiting hospital(t=5.38, p=0.000). Therefore, appropriate education programme depending on the education levels of patients should be established. It is proposed that nursing intervention is needed to assist the effective insulin self-injection through repeated substantial education.
